DaVita Integrated Kidney Care (IKC) seeks passionate, motivated Registered Nurse (RN) to join our team as a Remote- Hybrid Case Manager!
As an IKC Registered Nurse, Case Manager you assist some of the most complex patient population in navigating the health care system and making it work for them. You are able to support and look at a patient holistically, helping them and their whole care team, navigate the health care system. You are their advocate and get 1:1 time to assist in impacting the patient experience.
This is a field-based hybrid role supporting patients in the Glendale, CA market with both telephonic & in-person support pending patient needs.

This RN Case Manager functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESKD and CKD patients. This RN Case Manager provides coordination of care between the patient, family, physician, provider and care teams, the community, and Regional Operations Manager.

Qualifications for Registered Nurse, RN Case Manager :
A.D.N degree from accredited school of nursing required; B.S.N preferred
Current Registered Nurse (RN) license in the state of practice required
Continuing education credits maintained as required by state of practice required
Minimum of three (3) years' experience in clinical nursing required, Renal Nursing a plus
Armenian speaking preferred
Demonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomes
Experience and passion for educating patients
Current CPR certification required
Ability to adapt to new/changing technology
Current driver's license in state employed with positive driving record and able to meet requirements of insurance coverage required
Must be located in the Glendale, CA area
Hybrid position. Combination home office, visits to DKC Clinics/Nephrology practices
Basic computer skills and proficiency in MS Word and Outlook required
Home office with internet connectivity at minimum of 1MB upload and 1MB download speed required

$43.75-$53.08 / hourCompensation for the role will depend on a number of factors, including a candidate's qualifications, skills, competencies and experience and may fall outside of the range shown.
